- Access/Portability: Receive a copy of your data in usable format

- Restrict/Limit Use: Limit how we use your sensitive personal information
- Object: Object to certain types of processing
This form allows you to submit any of these requests. We will process your request in accordance with applicable privacy laws.
2. Categories of Requests Available
2.1 Right to Know / Right of Access
What You Can Request: A copy of what personal information we collect about you, including:
- Specific pieces of personal information we maintain
- Categories of personal information collected
- Sources of that information
- Purposes for collecting it
- Categories of third parties we share it with

2.2 Right to Delete / Right of Erasure
What You Can Request: Deletion of personal information we maintain about you. Important Exceptions: We may not delete information if:
- It's necessary to complete a transaction you initiated
- We need it to provide services you requested
- Deletion would breach our legal obligations
- It's needed for fraud detection or security
- Deletion would prevent detection of illegal activity
- It's required by law or regulation

2.7 Right to Object / Right to Restrict Processing (GDPR)
What You Can Request:
- Object to Processing: Object to processing based on legitimate interests, including marketing and profiling
- Restrict Processing: Request that we limit processing while we verify information or during a dispute

3. Information You Need to Provide
To submit a request, please provide the following:

- Email Address (where we'll send response)
- Phone Number (optional but helpful for verification)
- Type of Request (select one from Section 2 above)
- Description of Request (what data you want, or what correction you need, etc.)
- Your Signature (typed name acceptable)
3.2 Optional but Helpful Information
- Date range of relevant information
- Specific data categories you're interested in
- Whether you had a direct transaction with us (employment, form submission, etc.)
- How you'd prefer we deliver the response (email, mail, download link)
4. Identity Verification
Why We Verify: We verify your identity to protect your privacy and prevent unauthorized access to your personal information.
4.1 Verification Process
We may request you to verify your identity through one or more of the following:
- Email Verification: Confirm the email address associated with your account or our communications
- Phone Verification: Confirm phone number in our records or via callback
- Information Verification: Answer questions about information from our records (e.g., "Is your address [address]?", "Last 4 digits of phone?", "Services you've inquired about?")
- Government ID: For sensitive requests (deletion, financial info), provide a photocopy of government-issued ID
- Certified Verification: For some requests, notarized statement or certified copy
4.2 Response to Verification Failure
If we cannot verify your identity:
- We will notify you in writing (email or mail)
- We will explain what additional information we need
- You'll have 45 days to provide the verification
- If verification cannot be made, we'll deny the request with explanation
5. Authorized Agent Instructions
You may authorize a third party to submit a request on your behalf. The authorized agent must:

- Proof of Authority: Proof they have authority to act on your behalf
5.2 Verification Process for Agents

- May also verify your identity (the consumer's identity)
- Confirm the agent has the legal authority they claim
6. Response Timeline
6.1 California Residents (CCPA/CPRA)
- Initial Response: Within 45 days of receipt of verified request
- Extension: We may extend up to 90 days total for complex requests, with notice to you
- Communication: We'll respond to the email or address you provided
- Format: Right to Know responses provided in digital format or mail
6.2 European/EEA Residents (GDPR)
- Standard Response: Within 30 days of receipt of valid request
- Extension: May extend up to 60 days for complex requests, with notification
- Communication: Response to email or address provided
- Format: Data in structured, commonly-used format (CSV, JSON, etc.)
6.3 What Counts as "Receipt"
We consider your request "received" when it reaches us through any of these methods:

- Phone call (verbal requests must be confirmed in writing)
7. Non-Discrimination Statement
We will NOT:
- Deny you goods or services
- Charge different prices or rates
- Provide different service quality
- Suggest you'll be treated differently
- Retaliate in any way
The only exception is if we offer you a financial incentive for sharing data, which is voluntary and clearly disclosed.

For "Right to Know" requests, we will provide your personal information in one of these formats:

- Physical Format: Printed documents mailed to your address
- Secure Portal: Access to a secure online portal where you can download your data
- Email: Information provided directly in email for simple requests
10.2 Data Portability Format (GDPR)
For GDPR data portability requests, we'll provide data in a structured, commonly-used, machine-readable format such as:

- XML (Extensible Markup Language)
11. Request Status and Follow-Up
11.1 Acknowledgment
We will send you an acknowledgment email within 5 business days confirming receipt of your request and providing:
- Request ID number for reference
- Type of request received
- Expected response date
- Any additional information needed
11.2 Status Updates
For complex requests extending beyond 45 days (CCPA) or 30 days (GDPR), we will provide status updates every 15 days.
11.3 Request Denial
